All personnel are fully qualified and meet all state regulations governing their disciplines. All care rendered under physician’s orders.
Skilled Nursing Service:

• IV infusion therapy

• observations: vital signs, medications, post surgical, acute or unstable conditions

• catheter insertion and care

• wound care/dressings

• decubitus care

• venipunctures

• restorative nursing

• post cataract care

• bowel/bladder training

• administration of injections

• nasogastric tubes, colostomy, gastrostomy tubes, care of tracheostomy

• teaching of medications, diet, safety, disease process

• other care specific to your condition

Physical Therapy:

• therapeutic exercise

• transfer training

• gait training, ultrasound

• TENS Unit, prosthetic training

• balance exercise

• home exercise programs

Speech Therapy:

• a laryngeal speech

• language processing

• speech/voice

• swallowing

Occupational Therapy:

• home modification assessment

• training for activities of daily living

• adaptive equipment training

• upper extremity exercises

Medical Social Services:

• counseling

• financial assistance

• placement assistance

• arrangement for meals & transportation

• short-term home health aide

• personal care

• furnished under direct supervision of RN

• medical equipment and supplies

Personal Assistance Services:

Assistance to the client including assistance with the performance of the activities of daily living and household chores necessary to maintain the home in a clean, sanitary, and safe environment.
The level of assistance provided is determined by the client’s needs for assistance and the plans of care approved by the Texas Department of Human Services.

• To be available to respond to the physician in a timely manner.

• To submit written documentation and medical information to the physician in a timely manner which includes:

- plan of care
- clinical update summaries
- discharge summary

• To follow the plan of care as ordered by the physician.

• To notify the physician of changes in the patient’s status.

• bathing/hygiene/dress

• hair & nail care

• assist w/ elimination

• assist w/ ambulation transfers

• range of motion

• linen change

• light housekeeping pertaining to patient

• light meal preparation

• vacuum/mop entire house

• wash windows

• clean cupboards/closets

• lawn care

• transportation (unless prior arrangement made)

If uncovered services are needed, please notify the Agency or the nurse so an attempt can be made to assist with the need.

Referrals are accepted from friends, family, social workers, physicians or yourself. All referrals must be approved by your physician and a registered nurse and/or therapist will make an evaluation visit to your home. A plan of treatment is then developed, implemented, and reviewed every 60 days in consultation with your physician and staff of Amazing Grace Health Agency.

for services

• Medicare Part A – pays 100% for covered services – unlimited visits

• Medicare Part B – pays 80% for covered services – unlimited visits

• Medicaid – pays 100% for covered services

• Private Insurance/Workman’s Comp. – coverage varies with individual policies

• Private Pay
